What is Orlistat?

Orlistat is a lipase inhibitor — the same medicine as branded Xenical. Rather than changing your appetite, it works entirely in the digestive system: it blocks the enzymes that break down dietary fat, so around a third of the fat you eat passes through unabsorbed instead of being stored.

It's the affordable, needle-free option — a capsule taken with each main meal, no titration and no fridge. Average weight loss is more modest than with the injectable treatments, but it has decades of real-world use and works best alongside a reduced-calorie, lower-fat diet.

Most side effects are caused by the fat orlistat blocks leaving the body — keeping each meal below roughly 15 g of fat prevents most of them. A bedtime multivitamin covers vitamins A, D, E and K, and your pharmacist is a message away if anything feels off.

How much weight can I expect to lose on orlistat?
In a one-year trial, people taking orlistat 120 mg with a reduced-calorie diet lost around 10% of their body weight on average, versus about 6% with diet alone. That's more modest than the injectable treatments — your prescriber will help set realistic expectations. If you haven't lost at least 5% of your body weight after 12 weeks, treatment is usually stopped.
How do I take orlistat?
One 120 mg capsule with each main meal that contains fat — up to three a day, taken before, during, or within an hour of eating. If you skip a meal, or it's fat-free, skip that capsule too.
What's the difference between orlistat, Xenical and Alli?
They're all the same medicine. Xenical is the original brand of prescription orlistat 120 mg; generic orlistat 120 mg is identical and cheaper. Alli is the branded lower-dose 60 mg version — we supply that too, and it has its own page at pprx.co.uk/weight-loss/alli.
How does orlistat compare to Mounjaro or Wegovy?
It works completely differently — in the gut rather than on appetite — and average weight loss is smaller. Its advantages are price, decades of safety record, and no injections. Some people start here; others are better suited to a GLP-1. Your prescriber will advise what's clinically appropriate for you.
Do I need to change how I eat?
Yes — orlistat works best with a reduced-calorie, lower-fat diet, and spreading fat evenly across your meals. Very fatty meals are also what cause most of the digestive side effects. A daily multivitamin at bedtime is recommended, as orlistat reduces absorption of vitamins A, D, E and K.
Is orlistat available on the NHS?
Yes, subject to criteria — typically a BMI of 30 or more, or 28 with a weight-related condition. PPRX provides it privately following a clinical consultation, with pharmacist support throughout.
