BMI and age
Mounjaro, Wegovy and the Wegovy Pill are licensed in the UK for adults with a BMI of 30 or more, or 27 or more with at least one weight-related health condition. Our online service treats adults aged 18 to 65 with a BMI of 27 to 50.
BMI is your weight in kilograms divided by your height in metres squared. Our BMI calculator works it out for you. You'll be asked to confirm your height and weight during the consultation.
What weight is a BMI of 27 or 30?
Eligibility depends on BMI, not on weight alone, so the same weight can qualify one person and not another. The table shows roughly where a BMI of 27 and 30 fall at common heights. At or above the first column you need a weight-related condition as well; at or above the second, you don't.
Weight at a BMI of 27 and 30, by height| Height | BMI 27 at about | BMI 30 at about |
|---|
| 152 cm (5 ft 0 in) | 62 kg (9 st 12 lb) | 69 kg (10 st 13 lb) |

Weight-related conditions
With a BMI between 27 and 29.9, you also need at least one health condition linked to weight. The consultation asks about:
- Type 2 diabetes, or blood sugar that's higher than normal
- High blood pressure or high cholesterol
- Obstructive sleep apnoea
- Heart disease
- Polycystic ovary syndrome (PCOS)
- Fatty liver disease (MASLD)
- Osteoarthritis or chronic back pain
Who can't be treated online
A prescriber won't prescribe these medicines online if any of the following apply. Some are a risk with the medicine itself; others need care in person.
- You're pregnant, breastfeeding or trying to conceive
- You've had pancreatitis, or another condition of the pancreas
- You have gallstones and still have your gallbladder
- You have active cancer or are having cancer treatment
- You have, or have had, anorexia or bulimia
- You've had an allergic reaction to a GLP-1 medicine
- You have type 1 diabetes, severe liver or kidney disease, heart failure, diabetic retinopathy, a history of thyroid cancer, multiple endocrine neoplasia, a severe digestive condition, glaucoma, epilepsy or porphyria
- Your BMI is above 50, which needs assessment and support in person
If you aren't prescribed treatment
A UK prescriber reviews every consultation and can decline if a medicine isn't safe or suitable for you, or if they need more information first. Under our terms, if treatment isn't appropriate the order can be cancelled and refunded, subject to any administration fee that was clearly shown before you paid.
Being declined online doesn't mean nothing can help. Your GP can look at NHS weight management services and conditions that need care in person, and our coaching works without medicine. Please don't buy these medicines from a seller that skips a consultation: our guide to checking an online pharmacy explains the risks.
If you're already on treatment elsewhere
You can move to us from the NHS or another provider. Tell the prescriber which medicine and dose you take, when you last took it, and how you've got on with it. Under our terms the prescriber can ask for evidence of previous treatment, such as a prescription, a dispensing label or a message from your previous provider, and confirmation of your current medicines, weight and height.
The prescriber uses this to decide whether you can carry on at your current dose or need to step back down, for example after a break in treatment. Switching between Mounjaro and Wegovy has no official dose conversion: our dosing guide explains how switching works.
Can I take Mounjaro if I have thyroid problems?
An underactive thyroid that's treated doesn't usually rule out Mounjaro or Wegovy, but tell the prescriber about it and every medicine you take for it. If you take levothyroxine with the Wegovy Pill, your thyroid levels may need checking. Our online service can't treat anyone with a history of thyroid cancer or multiple endocrine neoplasia. The prescriber reviews your health history and decides whether treatment is suitable for you.
Our safety guide covers what the product information says about the thyroid.
Common questions about specific situations
If you have PCOS. PCOS doesn't rule you out, and it counts as a weight-related condition. The NHS, which now calls it PMOS, lists losing weight if you're overweight, and weight-loss medicines, among the treatments sometimes used. These medicines must be stopped before trying to conceive, so tell the prescriber if that's your plan. If you take the combined pill for your periods, Mounjaro can make it less reliable as contraception, so you may need an extra method for a while.
During or after menopause. The same criteria apply. Tell the prescriber about any hormone treatment you take.
If you take other medicines. List all of them in the consultation, including insulin or tablets for diabetes, which may need adjusting.
NHS and private eligibility
The NHS uses stricter criteria, set by NICE and introduced in phases, so many people who meet the licence can't yet get these medicines on the NHS. A private prescription follows the licence and the prescriber's judgement. Our guide to getting a private prescription explains how it works, including whether your GP needs to know.
Who can get Mounjaro on the NHS
On the NHS in England, tirzepatide (Mounjaro) is being introduced in phases over three years, under NHS England's interim commissioning guidance for NICE's TA1026. Eligibility depends on your BMI and how many of these conditions you have: high blood pressure, abnormal blood fats (dyslipidaemia), obstructive sleep apnoea, cardiovascular disease and type 2 diabetes.
NHS tirzepatide phases in England| From | BMI | Listed conditions |
|---|
| Now (first phase) | 40 or more | 4 or more |
|---|
| Around June 2026 | 35 to 39.9 | 4 or more |
|---|
| Around March 2027 | 40 or more | 3 |
|---|
Semaglutide (Wegovy) on the NHS is under NICE TA875: only through a specialist weight management service, for up to 2 years. Source: NICE TA1026 and TA875.
Do you need diabetes?
No. Mounjaro and Wegovy are licensed for weight management whether or not you have diabetes. Type 2 diabetes and prediabetes are among the weight-related conditions that count if your BMI is between 27 and 29.9.
Ozempic is a different case. It contains semaglutide, the same medicine as Wegovy, but in the UK it is licensed only for type 2 diabetes, at doses up to 2 mg a week. For weight management, the semaglutide medicine is Wegovy.
If your BMI is under 27
Mounjaro, Wegovy and the Wegovy Pill aren't licensed for weight loss below a BMI of 27, so a prescriber can't supply them to you for that. Orlistat isn't an option either: the 120 mg capsules are licensed from a BMI of 30, or 28 with other risk factors, and Alli from a BMI of 28.
The NHS has free support for healthy eating and activity, and our coaching works without medicine. If your BMI changes, you can check again with our BMI calculator.
What happens next
The online consultation asks about your health and takes a few minutes. A UK prescriber reviews every answer and can ask you questions or decline. If treatment is suitable, the comparison of Mounjaro and Wegovy and our pricing page explain the options.
